South Lanarkshire Community Health Partnership
Community Health Partnerships, or CHPs for short, are the new organisations which are being developed across Scotland to manage a wide range of community based health services. CHPs are designed to further improve the way your local community health services are organised and managed and to develop closer partnerships between community health, social care and hospital based services. They will build on the work of the Local Health Care Co-operatives (LHCCs), which were previously responsible for managing local health services, but are larger organisations with wider responsibilities and greater influence.
CHPs will break down barriers that have traditionally existed between health and social care to make it easier for you to access the local services you require, regardless of whether these are provided by your health centre, local council or hospital. CHPs will also develop strong links with local GPs, dentists, pharmacists and opticians and other services such as housing, leisure and education to help improve the health of local people.
Two CHPs have been developed across NHS Lanarkshire - one covers the area served by South Lanarkshire Council including Rutherglen and Cambuslang, East Kilbride, Hamilton/Blantyre and Clydesdale.
As the Rutherglen and Cambuslang area lies within the NHS Greater Glasgow and Clyde boundary, NHS Greater Glasgow and Clyde will work closely with NHS Lanarkshire to deliver local health services across this area.
