District Nursing
The District Nursing Service is part of a range of domiciliary services available to the residents of the Renfrewshire Community Health Partnership (RCHP) area which provides nursing care to adults over 16 years of age with a wide range of conditions, illnesses and health needs. Working collaboratively with other service providers the District Nursing Service aims to provide professional, evidence based nursing care to patients in their own homes.
Home may also refer to hostel, hotels and care institutions where nurses or carers’ with the appropriate skills necessary to meet the patients’ needs are not available. District Nurses also assisit in Health Promotion and Health Improvement initiatives, in particular where the patient is required to change lifestyles, which may have an impact on their medical condition.
District Nurses are aligned to General Practices where they are part of a larger Primary Health Care Team. The District Nurse/Charge Nurse aligned to a GP practice is a specialist practitioner who has undergone post registration training. There will be a skill mix within his/her team, which will include staff nurses and healthcare assistants.
Planning care – this is the process which is decided in collaboration with patients’, carers, statutory agencies and other informal agencies where appropriate. The aim is to promote patient independence whilst ensuring risk to their safety is minimal, throughout the 24 hours of the day. For those patients in hospital with complex needs a case discussion should be arranged and documented with key responsibilities highlighted to minimise risk to patients’ on discharge.

Palliative and terminal care.
Ambulant patients will receive their health care needs within GP surgeries.
Joint Equipment Store (EquipU)
Assessment for specialist beds, pressure relieving equipment, moving and handling equipment and commodes will be carried out by the District Nursing Team and provided by EquipU.
