Keep well Project
Keep well (formerly Prevention 2010) is a Scotland wide initiative who’s target population is patients aged between 45-64, attending participating GP Practices.
What is Keep well?
The Keep well vision is ‘to increase the rate of health improvement in deprived communities by enhancing primary care services to deliver anticipatory care’.
How we will deliver:
We identify, contact and engage all those in the target population
We offer risk assessment and effective treatment for Cardio Vascular Disease risk factors to those in the target population
We refer to those in the population to other NHS and community services as and when required and target those at particular risk of preventable serious ill health (including those with undetectable chronic disease).
Follow-up patients in the target population in order to monitor progress and enhance compliance and adherence with treatment.
Individuals within the target population receive a letter or phone call from their GP Practice inviting them to attend a Keep well health check. The health check is essentially a risk assessment carried out by healthcare practitioners to identify intermediate clinical factors as high cholesterol, lifestyle risk factors i.e. smoking and any other issues that may impact on health.
Based on assessment, individuals are offered or directed to appropriate services or support. This may include healthy eating advice, physical activity classes, smoking cessation, alcohol interventions, money advice, employability options and a selection of stress management interventions.
