West Glasgow Community Health and Care Partnership (CHCP for short) is a partnership between Glasgow City Council and NHS Greater Glasgow and Clyde.

It is one of five area-based partnerships across Glasgow introduced to manage local community-based health and social care services.

The CHCP will improve the way your Primary Care, community health and social care services are organised and delivered. These services will continue to be provided in local health centres, clinics, social work offices and community settings across West Glasgow.
