REHABILITATION SERVICE WITHIN GLASGOW
The Glasgow City CHP’s Rehabilitation Service provides specialist rehabilitation to adults over the age of 16 years with complex health needs. The service provides coordinated interdisciplinary assessment and treatment in response to community referrals, and to support hospital discharge.
The service brings together:
• Supported Discharge (DART, IRIS, MATCH)
• Community Physical Disability Teams
• Community Older People’s Teams
Patients referred will be at risk of deterioration in function if rehabilitation is not provided within the community or the patient will benefit from early supported discharge, having ongoing nursing and rehabilitative needs, which can be met at home as an alternative to remaining in hospital.

Renfrewshire
With the implementation of the rehabilitation and enablement service we will continue to operate a S ingle Point of Access in Renfrewshire and all referrals previously made to Older Adults Community Mental Health Team (OACMHT), Multi Agency for Care at Home Team (M.A.T.C.H), Specialist Nursing Service, Domiciliary Physiotherapy, Adult Speech and Language and Community Physical Disability Service are now made through our SPOA .
